Thank you for considering our study!

You are invited to be in a research study. Taking part in a research study is voluntary. Before you decide, you should know why the research is being done and what it involves. Please read this form carefully. Please take your time to decide. Before you sign this e-Consent you can call the research staff at this number: (845) 398-2183 or the Principal Investigators of the study with any questions you may have. You may download a copy of this form to read again. Being in a research study is not part of your child’s routine medical care. You can decide not to be in this study.

Before you continue reading it is important for you to know that you are being asked to participate because your child has completed other studies with one of the investigators in the past and agreed to have us contact you again. We would like to ask you additional information that relates to the coronavirus outbreak and how your child has been affected by it. We will be asking you a series of questions in online surveys about your child’s experiences, mood, and behavior in relation to this public health crisis.

There is no direct benefit to your child for participating in this study, but we hope the information collected will benefit others in the future by advancing understanding of how emergency situations affect mental health and life circumstances.
